TEACHER'S AGREEMENT
GRADES 9-12
We (I), _______________________________________________ and/or ______________________________________________, the parent(s) of the following student(s):
___________________________________________________________________________________________________________,
promise to uphold the policies of the Christian Institute of Arts & Sciences, Inc., namely:
1. I have read the CIAS High School Handbook.
2. I will have all shot records up to date and sent into the school office by October 1.
3. I will provide the school with pertinent information concerning students; i.e. adoption papers, custodial papers, etc.
4. My student will receive an Curriculum Consultation as directed by the administrator at the beginning of each school year.
5. I will acknowledge administrator's suggestions on curriculum planning, according to a student's learning style/needs.
6. I will attend any meeting or consultation that CIAS deems mandatory or necessary.
7. I will teach the required 180 days of school per year and/or complete the curriculum.
8. I will teach the required number of hours pertaining to grade level per day.
9. I will teach minimum subject areas of Bible, math, English (Language Arts), history, and science.
10. I will keep a record of daily attendance and schoolwork grades of each student I teach.
11. I will submit the quarterly report of daily attendance and a record and average of grades and to pay late fees for all late reports.

I will make all payments for registration and tuition to the school for
the months of August through July and to pay late fees
for all late payments and reports.
13. My student(s) in grades 2-12 will receive achievement testing every year, monitoring their academic and cognitive achievement.
14.
I will provide adult supervision for children up to and including the
age of 15, during school hours during the school year,
knowing that failure to do so constitutes child
neglect under compulsory education laws.
15.
I will prevent students from indulging in time-wasting activities during
school hours, including, but not limited to, talking on the phone,
shopping, all
non-educational internet activities, foolish behavior, etc.
16. I understand that failure to uphold the CIAS Conduct Code with my student may incur disenrollment from CIAS.
17. I will inform school administration immediately in the event that any student is apprehended or arrested by law enforcement authorities.
For high school students only:
17. I will follow the state of Florida's and CIAS's course requirements to receive credits.
18. I will provide documented records
of electives and extra curricular activities; i.e. physical fitness, community
service, performing fine
arts (theatre, drama, music lessons, choir, etc.), in
order to receive credit and to substantiate grades on the quarterly reports.
19. I will attend the school's high school orientation and pertinent meetings with high school students.
20. I
will abide by the Student Conduct Code, as given in the CIAS High School
Handbook, knowing that failure to do so will result
in my disenrollment from
CIAS.
We/I further agree that if we/I wish to resign as teacher(s) from CIAS, we/I agree to:
A. Immediately contact the school and inform the administrator of intent to resign.
B. Mail all remaining forms and information on our student(s) to the school office within ten days.
C. Fulfill all policies and requirements regarding dismissal or withdrawal from the school.
We (I) understand that failure to comply with the school regulations and policies are cause for teacher certification removal and dismissal of student(s). School records will then be stamped incomplete.
We (I) release the Christian Institute of Arts & Sciences, Inc. from any and all responsibility and absolve them from any claim of loss, damage, or injury of any nature to person or property resulting from the schooling program. We (I) also agree that the school shall not be liable for any loss or unintentional neglect or careless acts of any school personnel or other enrolled families and students.
